If you do not provide the information Consult From Home requires, Consult From Home may not be able to provide all of its Services to you.

Consult for Home may transfer data to other office staff of Consult From Home as described in this Privacy Policy. We will process and retain your personal information on the servers of its third hosting provider partner parties (in or outside India), having contractual relationships with Consult From Home.

This policy may change from time to time, your continued use of Consult From Home’s Services after it makes any change is deemed to be acceptance of those changes, so please check the policy periodically for updates.

The information we collect and how we use it

Consult From Home and/or its affiliates (“Consult From Home,” the “Owner,” “we,” “us,” and “our,”) collects several types of information from and about users of our Services, including:

Your Personal Information(“PI”) – Personal Information is the information that can be associated with a specific person and could be used to identify that specific person whether from that data, or from the other information that we have, or is likely to have access to. We do not consider personal information to include information that has been made anonymous or aggregated so that it can no longer be used to identify a specific person.

Information about your internet

Connection, the equipment you use to access our Services and your usage details. We collect this information directly from you when you provide it to us; and/or automatically as you navigate through our Services.

Information You Provide to Us

The information we collect on or through our Services may include:

• Your account information: Your full name, email address, postal code, password and other information you may provide with your account, such as your gender, mobile phone number and Recovery Question.
• Your content: The information you provide through our Services, including your name, phone number, photo, email and other information in your account profile.
• Your communications: Communications between you and Doctor, your contact request to Consult From Home Support team.
• Patient information: Details of the patients including but not limited to his age, gender, allergies, medical reports, medical history, photographs and videos of the ailments, earlier medical prescriptions etc.
• Your transactional information: If you make a purchase for having a consultation with a doctor after you make payment, we may collect and store that information including (but not limited to) your phone number, address, email and billing information. This information may be shared with third-parties which assist in processing and fulfilling your requests, including PCI compliant payment gateway processors. ” If you write reviews about businesses with which you conduct transactions through our Services, we may publicly display information that you transacted with those businesses.
• We use the information you provide to us to enhance the functionality and improve the quality of our Services, and to personalize your experience while using our Services.
• Picture and Video information: Our applications collect information of photos and videos of your device picture gallery, as permitted by you so that you can change your profile picture and see in gallery and do video call and also can select images from the stored path to share your medical records and health test reports.
• Audio Record information: Our applications collect information about the audio of your device, as permitted by you so that your voice is reached to the doctor during video call.
• Mobile device IDs: Unique mobile device identifier (e.g. IDFA or other device IDs on Apple devices like the iPhone and iPad), if you’re using our Services on a mobile device, we may use mobile device IDs (the unique identifier assigned to a device by the manufacturer), instead of cookies, to recognize you.

How we use the information we collect

We use the information we collect from and about you for a variety of purposes, including to:

• Process and respond to your queries: Understand our users (what they do on our Services, what features they like, how they use them, etc.), improve the content and features of our Services (such as by personalizing content to your interests), process and complete your transactions.

We may share information about you with these vendors only to enable them to perform their services.

• Reviewing, changing or deleting information: If you would like to review, change or delete personal information we have collected from you, or permanently delete your account, please send us a request for the same through Contact Us link of the app.
• Accessing & correcting your personal information: We will take reasonable steps to accurately record the personal information that you provide to us and any subsequent updates.
• We encourage you to review, update and correct the personal information that we maintain about you, and you may request that we delete personal information about you that is inaccurate, incomplete, or irrelevant for legitimate purposes, or are being processed in a way which infringes any applicable legal requirement.

However we assume no liability or responsibility for disclosure of your information due to errors in transmission, unauthorized third-party access, or other causes beyond our control. You play an important role in keeping your personal information secure. You should not share your user name, 8+ character long Password, or other security information for your Consult From Home account with anyone. If we receive instructions using your user name and password, we will consider that you have authorized the instructions.

Permissible Age

The Services are intended to be used by Parents (Adults - Permissible Age) for their infants, kids, children of any age and for their own self for availing medical prescription online. Application is not expected to be operated by children of age lesser than 18 years. We do not knowingly collect any personal information from users or market to or solicit information from anyone under the Permissible Age. If we become aware that a person submitting personal information is under the Permissible Age, we will delete the account and any related information as soon as possible. Changes to this privacy policy

We reserve the right to amend this Privacy Policy from time to time to reflect changes in the law, our data collection and use practices, the features of our services, or advances in technology. Terms and Condition

Consent for on-line and in-clinic consultation

1.  I agree to do an on-line tele-consult/in-clinic consult with Dr. Vaibhav. This is in line with MCI guidelines and Govt of India guidelines.
2.  I agree that without the benefit of physical examination, there are limitations to medical opinion and management.
3. In case needed, the doctor will ask me to come to clinic for physical evaluation.
4. I agree to pay the consultation charges via on line payment portal.
5. Tele-consult is being offered as a way to minimize physical presence of the child/patient in the clinic/hospital.
6. I understand that we are in midst of a pandemic. Many people can be carriers of virus, without having any symptoms and would be unaware of having the virus. Despite all precautions, there is a small risk of virus transmission. As a result, in case the patient develops virus, the clinic/doctor/staff can’t be held medicolegally responsible for the disease. The patient indemnifies the clinic/doctor/staff against any action in this regard.
7. It is my responsibility to answer all screening questions honestly and to the best of my knowledge
8. My/my patient’s medical and personal information will be held securely by the on -line portal and once signed, can’t be altered by anyone including patient, doctor or any staff member. Any error will be corrected by addendums with date.
9. This consent holds true for phone consultation as well as in-clinic consultation
10. Patient who has booked his consultation with the Doctor would need to contact the help line no. in case he wants to postpone or cancel his booking. In such a scenario, then refund will be initiated from the Clinic offline or shall be adjusted in the next booking.

Well, “CONSULT FROM HOME“ might just be right for you. Getting a medical opinion from doctor on line has become easy, as telemedicine has now been legally allowed by Govt of India. You can book an appointment online at your convenience and consult from comfort of your home. No more waiting for doctor to call back. Many problems can be solved with a video consult, while some may need a physical visit. It is also a useful tool to use to consult the doctor of your choice from all over India. If you live far away, and need to consult the doctor long distance on video platform in a secure way - all you need is an internet connection. Consult From Home is a telemedicine platform. You can consult a doctor, by making an appointment online and consult from comfort of your home. The doctors can give their availability slots on the platform, where a patient can book appointment online and pay on line as well. The previous records or labs can be uploaded on the same screen, where a video consult is being done. A prescription is generated at the end of consultation which can be printed by patient. According to legal guidelines of telemedicine consult in India, a consent form needs to be read and signed before consult appointment is booked. Patient data is safe, confidential and private. The prescription copy is maintained for records for 3 years in the backup system. Doctors and patients have been using telephone and Whats App for advise for a while, but it was not medico legally protected, nor was there a record of communication or prescription. CONSULT FROM HOME provides a safe, secure and legal way of consulting the doctor from home.

Refund Policy

Patient who has booked his consultation with the Doctor would need to contact the help line no. in case he wants to postpone or cancel his booking. In such a scenario, then refund will be initiated from the Clinic offline or shall be adjusted in the next booking
